  • Yes. If you or a loved one is interested in receiving home health care services, speak with your primary care physician, nurse practitioner or other health care provider. You can also talk to one of our nurses 24/7 at (980)346-9507 to find out if home health might be right for you.

  • We need a physician's order outlining the services needed for the patient, a history and physical of the patient, the notes from the patient's most recent interaction with their physician and their demographic and insurance information.

  • Yes, we do. We’ll work closely with your physician or other authorized practitioner to develop and strictly follow your plan of care.

  • Your parents or other loved ones may benefit from home care if they exhibit any of these signs:

    • Decline in personal hygiene

    • Lost interest in cooking

    • Diagnosed with Alzheimer’s or another form of dementia

    • Forgets to pay bills

    • Forgets to take medications

    • Needs help getting up and down from chairs or sofas

    • Falls frequently

    • Has limited mobility

    • Has a chronic disease like arthritis that makes tasks like opening jars or buttoning a shirt difficult

    There is no “one size fits all” answer. We would encourage you to contact our offices at (980)346-9507 for us to develop a personalized care plan for your needs.

  • We try to start our patients within 24 to 48 hours after receiving the referral from the health care provider. There are exceptions if there's a delay in the start of care due to a patient's request, a pending surgery or a patient who has not yet been discharged from the referring facility.

Ways You Can Pay

  • Medicaid is a state government program that provides health care insurance and medical assistance to people, including families and children, who meet income eligibility requirements.

  • Veterans benefits includes a wide range of services including long term care, home or community based care, and nursing home or facility care. Your eligibility for these services will be determined based on your need for ongoing treatment, personal care, and assistance, as well as the availability of the service in your location. Other factors, such as financial eligibility, your service-connected (VA disability) status, insurance coverage, and/or ability to pay may also apply.

  • In the event you choose to pay for home health care services without the assistance of health insurance, Assisting Families Home Health Care is happy to work out payment terms with you.

  • CAP/DA is a Medicaid Home- and Community-Based Services and Community-Based Waiver Services. The waiver program provides a cost-effective alternative for a Medicaid beneficiary who is medically fragile and at risk for being required to leave the care of their own personal home.   • Personal Care Services (PCS) provides personal care services to individuals residing in a:

    Private living arrangement

    Residential facility licensed by North Carolina as an adult care home

    Combination home or Group home

  • The Program of All-Inclusive Care for the Elderly (PACE) is a managed care program for older adults. This program features a comprehensive service delivery system, and integrated Medicare and Medicaid financing.
